Integrity Well-Being, powered by Amaze Health
A wellbeing program, not another health plan
Integrity is a wellbeing program powered by Amaze Health. It doesn’t replace your health plan or the doctor your employees love. It works right alongside them. It has three parts:
- Amaze Health care. Real doctors and clinicians your employees can reach anytime, for the whole household, even an elderly parent living with them. Unlimited visits, no copays.
- Accident and hospital coverage. Pays set amounts for covered accidents, directly to the employee. For example, on one plan: $500 for an ambulance ride or $650 for an emergency room visit. Claims go through the same Amaze team.
- A monthly wellbeing benefit. Each month Amaze sends health education made for the employee, and as an enrolled member they receive a monthly wellbeing benefit.
How it works with the paycheck
Like a health plan, a 401(k) or an HSA, the program cost comes out before taxes, so employees pay less in taxes and keep a little more.
Michigan example: an example plan costs $1,500 a month, and the monthly wellbeing benefit is $1,200 a month. Because the cost is pre-tax, take-home pay can go up. Illustrative only; results vary.
- No surprises. An employee is enrolled only if their take-home pay goes up, and each gets a paycheck example made for them.
- Time off? No paycheck means nothing comes out and the benefit pauses; Amaze care keeps going for 60 days, and re-enrollment is automatic on return.
- Easy opt-out. Employees learn about it first and can opt out with one call to Amaze Health.
- If they leave, their Amaze care team can come with them as a low monthly membership.

Employers with roughly 25 or more employees who are mostly full-time W-2 staff with steady tenure. Your role is payroll administration; your advisor walks you through setup and the timeline for your payroll system.
Smaller or mixed team: Amaze Direct
Part-time, seasonal, 1099 or high-turnover crews where payroll mechanics aren’t a clean fit can add Amaze Health as a flat employer-paid monthly subscription, quoted for your group. Owners and 1099 workers can also join on their own for about $49 a month.
